Definitions

“The Company” Lazy Frog Floatation Centre
“The Buyer” The buyer of goods from the company
“The Client” A person who books to attend an appointment face to face
at the clinic in Swansea UK.
It is your responsibility to make yourself aware of our Terms & Conditions prior to booking an appointment or purchasing a gift voucher.
Please read these Terms and Conditions carefully as they contain important information regarding your rights and obligations as a Buyer / Client. By booking or purchasing a gift voucher, you are fully agreeing to our Terms & Conditions.
Lazy Frog Floatation Centre is based in Swansea UK and any appointment or Voucher used for a treatment must be booked in advance by contacting us – 01792 476307

Code of Professional Ethics

During any treatment the Therapist will ensure to keep the client informed of all methods that will be employed during the session.
Ensure conduct of all business and professional activities with honesty and integrity.
Any person under 18 years of age must be accompanied by a parent or guardian who is required to sign a disclaimer on the 1st visit to the clinic for any treatment provided and a parent /guardian may be required to attend for further appointments.

Customer Code of Conduct

We only offer professional treatments. If during a treatment any inappropriate or lewd conduct is suspected, the therapist will stop giving the treatment and the client will be requested to leave the site. Full payment will be due for the treatment and no refunds will be made.
Late attendance / Appointments not attended without prior arrangement (No Show):
Fees are payable in advance of each appointment. Please be aware that we need to be informed if you are unable to be punctual. Late arrival for an appointment may lead to a shorter treatment, or loss of appointment, and the full appointment fee will be payable by the
Client. We cannot keep your appointment for any longer than 30 minutes if you are late your appointment will be cancelled and you will be responsible for the full amount of the booked treatment.
In the event of a No Show you will be responsible for the full amount of the booked treatment.

Cancellations Policy

We require a minimum of 24 hours notice to cancel or change an appointment. If the appointment is cancelled by the client with less than 24 hours notice, the Client will be charged for the full amount of the booked treatment.
Where an email, answer phone message, or any other notification to inform the therapist that an appointment is to be cancelled has been left, it is the Clients responsibility to confirm that the therapist has received this information, by receiving a confirmation message in return, or speaking to the therapist directly. If no confirmation message has been returned to you the full amount of the booked treatment will be charged.

Recovery of outstanding accounts by invoice

If we have to pursue an unpaid invoice or cancellation fee, the client will be liable for all additional administrative ad recovery costs incurred by Lazy Frog Floatation Centre and or a third party debt recovery agent, in recovering payment.
An invoice letter requesting payment within 7 days will be sent and, if necessary a further letter requesting the client to resolve their unpaid account will be sent. Following this second letter, all further correspondence will incur an administration fee of £10.00 per correspondence. Any additional recovery charges will be added to the amount to be recovered.

Gift Cards/ Vouchers

We will offer a refund within 28 days of purchase, after this date, vouchers are non-refundable but may be transferred to someone else. Gift cards / vouchers bought for treatments are valid for 3 months and cannot be extended. Out of date vouchers are donated to local schools and charities for use as raffle prizes.

Client Consultation and Confidentiality

If you have any medical condition or concerns regarding your treatments please contact us in order to discuss this prior to your visit. All personal details provided will be kept in the strictest confidence as stated within the Data Protection Act.

Age

You must be 18 years of age to book any appointments. We have the right to ask for your ID even if you are over 18 years of age. If you are under 18 and would like to receive treatment your parent or guardian must attend with you and sign our disclaimer.

General

All sales are subject to these terms and conditions and no other shall apply. Placing an order on our website or by phone constitutes acceptance of our terms and conditions of sale. We reserve the right to make alterations or update our terms and conditions as necessary. All alterations will apply as soon as changes are made.

Membership

Membership is for a minimum of 6 months and can be terminated anytime after this by giving us one month notice.
If a Member is unable to float with us one month, they may roll that over to use the following month.
The maximum number of floats a member may have to use at any one time is two.
Outstanding float sessions can only be used while membership is still active. Termination of membership will result in the loss of any outstanding floats.
